Answer first: before publishing or refreshing an HSE-linked page, open every official source, confirm the domain and final URL, compare the page’s exact scope, record the review date, remove unsupported service promises and keep a visible independent-site disclaimer.

Change triggers
Recheck immediately if a user reports a wrong contact, a source redirects, a clinic page changes, the HSE changes a service description, a linked provider changes its eligibility or privacy information, or a page’s health claim cannot be traced. Otherwise, sensitive source-navigation pages should receive a scheduled review rather than being treated as evergreen.
Correction record
Keep the old wording, new wording, source URL, date checked and reason for the change. Correct only the supported point. Do not use a contact correction as permission to rewrite clinical content.
